By checking out I agree to take full responsibility for myself as I use the mild hyperbaric oxygen chamber and oxygen concentrator (if applicable) supplied by OxyLife and I assume all risk unto myself. Additional members listed for the treatment will be obliged to sign this waiver prior to using the mild hyperbaric oxygen chamber and oxygen concentrator (if applicable) presented by OxyLife.
I declare myself physically and mentally capable to participate in the sessions and declare to follow the safety instructions provided at all times. I will not misrepresent myself to any staff/agents about my ability to undergo mHBOT sessions at home. I understand that I am using mHBOT at the location specified for the rental.
I understand that this written waiver serves as the primary source of information and education of the use of mHBOT (Oxynova 7 and Oxynova 8). I agree that this service is not used to cure or treat any disease and is neither implied nor should be inferred. I am responsible to speak to my doctor should I have any concerns about undergoing mild hyperbaric oxygen treatment.
I agree to consult a physician/doctor if I am concerned about the activities/services OxyLife and how it relates to my health. I understand that if I have a medical condition of any sort or if I do not, that I accept full responsibility and OxyLife accept no liability and is not responsible for any damage or harm caused by the use of its products/services, activities.
